Traveling mentor expense report

Please submit this report after completion of a mentorship in which you travel to the student/s. PHCP will pay the Mentor’s round trip airfare or auto mileage. Students will be responsible for remaining expenses including food, lodging and any ground transportation. PHCP subsidies do not apply to traveling mentorships fees. Once submitted, you will be reimbursed.
