Clinician expense report

Please submit this report after completion of a PHCP clinic or group of clinics. You may add expenses and receipts to this online form and save it as a Draft until submission. If you have been previously reimbursed for an item related to a clinic, please include it with the receipt and check the already reimbursed box so it won’t be included in the total. Once submitted, you will be reimbursed for the total expenses and paid $155 per clinic attendee, per day.
