Terms & Conditions
Progressive Hoof Care Practitioners (PHCP) will pay the undersigned presenter the agreed-upon conference presenter fee and expenses, to be indicated below.
Location: Crowne Plaza Denver Airport Convention Center, Denver, Colorado
Presentation date/time: Sunday, October 9, 2022, 8:00am - 2 hours of presentations with a break.
Presenter fee: $1,400.00
Expenses include: airfare to be reimbursed, there is a free shuttle to the hotel, Saturday hotel room includes breakfast arranged and paid for by PHCP,
Upon receipt of this signed contract, 50% of the Presenters fee will be paid via check, PayPal or Venmo. Please indicate your preference below.
Expenses and the Presenter fee balance will be paid at the conference by the method of your choice.
My signature below indicates acceptance of the terms and conditions related to my participation as presenter at the 2022 PHCP conference.
